Trainee Onboarding Tools
The UW LEND program requires completion of onboarding steps prior to participating in the program. Completed materials are securely filed by the UW LEND Program Assistant. There are two categories.
UWMC Clinical Requirements
UWMC Clinical Forms are required by the University of Washington Medical Center (UWMC) to participate or observe in CTU/LEND Clinics.

UW LEND Requirements
UW LEND Forms are required for participation in the UW LEND program.
*Why make a slide? You will prepare a slide to introduce yourself to fellow trainees and faculty. Your slide will include a photo and information about yourself.
